Sport and Spine Rehab will fax you:

• The report of the initial evaluation

• Progress notes

• Discharge notes

You will be notified if the injured worker

• Cancels or No Shows for an appointment

• Is not compliant with treatment recommendations

• Shows lack of effort or willingness to participate

• Demonstrates "Red Flags"

• Demonstrates functional abilities that are sufficient for return to work

• Is capable of advancement with therapeutic activities
